As of now, the add-on may not work well with recent Anki versions, will be updated in the future.

The add-on works only on Windows. After installing the add-on, right click on a picture in the Editor window, select Edit and choose a default image viewer, e.g. FSViewer.exe (FastStone Image Viewer). Resize, edit or crop the picture, close FastStone Image Viewer and the changes will be visible in the Editor.
The edited image will not be synced with AnkiWeb right away. The issue might be fixed some time later.
For Mac or Linux, a picture will be opened with the default image viewer, but the changes will not be visible until the Editor window is closed or maybe Anki is restarted.
The add-on can be considered as a simple version of OpenInExternalEditor by ijgnd with “image_edit_externally__block_Anki_during_edit” set to True on Windows and it might be better to use OpenInExternalEditor instead.
